Three-year follow-up of a multicenter phase 2 trial of neoadjuvant atezolizumab plus carboplatin and nab-paclitaxel for stage IB-IIIA non-small cell lung cancer found that median disease-free survival and overall survival were 34.5 months and 55.8 months, respectively, with landmark overall survival at 36 months of 77%. Among patients with recurrence, 43% had brain metastasis. Patients whose tumors had mutations in STK11 and KEAP1 did not have a significantly higher incidence of brain metastasis. Reduced copy number of STK11 was significantly associated with worse pathological response and incidence of brain metastases. The authors conclude that among patients with stage IB-3A, the neoadjuvant combination of atezolizumab plus chemotherapy was associated with prolonged disease-free survival and overall survival.
