The oncological safety of fertility-sparing surgery (FSS) in young women with early-stage cervical cancer is not known. A nationwide retrospective matched cohort study in The Netherlands compared FSS versus radical hysterectomy among women aged 18 to 45 years with FIGO IA1 to IB2 cervical cancer. During median 8.9 years of follow-up, the FSS group had an elevated risk of recurrence (7.1% versus 3.6%; HR 2.21; 95% CI 1.38-1.80). The overall survival did not differ between the groups (HR 1.06; 95% CI 0.62-1.80).
