Adrenocortical carcinoma (ACC) has a poor response to systemic chemotherapy. Cabozantinib is a multikinase inhibitor approved in multiple malignancies. A phase 2 trial at MD Anderson Cancer Center (Houston, TX) evaluated cabozantinib monotherapy in patients with advanced ACC. Median follow-up was 36.9 months (IQR 30.2-50.3). The 4-month progression-free survival rate (primary end point) was 76%. The median PFS and OS were 6 months and 24 months, respectively. Treatment related adverse events of grade 3 or worse occurred in 61% of patients. There were no grade 5 TRAEs. The authors conclude that cabozantinib monotherapy had promising efficacy and manageable safety in patients with advanced ACC.
