Single-center phase 1 trial of anti-BCMA/GPRC5D bispecific CAR T cells for relapsed or refractory multiple myeloma

A phase 1 trial at the Affiliated Hospital of Xuzhou Medical University (Xuzhou, China) evaluated bispecific anti-BCMA/GPRC5D bispecific CAR T cells in patients aged 18-75 years with relapsed or refractory multiple myeloma (RRMM). The maximum tolerated dose was identified as 2.0 x 106 CAR T cells per kg. The most common grade 3 or worse adverse events were hematological toxicities (90% of patients). CRS was observed in 71% of patients; all cases were grade 1 or 2. No ICANS was observed in patients receiving 0.5-2.0 x 106 CAR T cells per kg. The overall response rate was 86%, with 62% having a complete response or better, and 81% having measurable residual disease negativity. Among patients receiving 2.0 x 106 CAR T cells per kg, the overall response rate was 92%, with 75% of patients having a complete response or better. The authors conclude that anti-BCMA/GPRC5D bispecific CAR T cells showed a good safety profile and encouraging activity in RRMM patients.
