Safety and feasibility of chemotherapy followed by liver transplantation for patients with unresectable colorectal liver metastases

The 5-year survival rate for patients with unresectable colorectal liver metastases (CLM) is around 10%. The randomized TransMet study (20 centers in France, Belgium, and Switzerland) compares chemotherapy followed by liver transplantation versus chemotherapy alone for these patients. Among 157 patients submitted to the validation committee of the study 63 patients (40%) were not randomized because of tumor progression (n=50) or potential resectability (n=13). Among patients randomized to the liver transplantation arm, 19% did not undergo transplantation because of disease progression while on the waiting list. Results of the study are expected to be published in 2027.
