A real-world retrospective study by the U.S. Myeloma Immunotherapy Consortium evaluated teclistamab for RRMM in a cohort of 509 patients, 89% of whom would have been ineligible for the MajesTEC-1 trial, primarily due to prior BCMA-directed therapy, cytopenias, or poor performance status. Patients had received a median of 6 previous lines of therapy (range 2-18) and started teclistamab at a median of 5.8 years (range 0.4-28.6) after their MM diagnosis. CRS was seen in 54% (1.4% grade ≥ 3) and ICANS in 11% (2.2% grade ≥ 3). Infections occurred in 42% and contributed to death in 5%. Partial response and very good partial response were achieved in 53% and 45%, respectively. The estimated progression-free survival was 5.8 months, and the 12-month overall survival rate was 61%. The authors conclude that teclistamab was tolerated and had activity for RRMM in real-world patients.
