Retrospective cohort study of survival impact of adjuvant chemotherapy after preoperative FOLFIRINOX for localized pancreatic adenocarcinoma

The overall survival (OS) impact of adjuvant chemotherapy after preoperative (m)FOLFIRINOX and resection of localized pancreatic adenocarcinoma is not clear. A multinational retrospective cohort study of 767 patients found an association between adjuvant chemotherapy and prolonged OS in all subgroups, which could be less in patients receiving 8 to 11 preoperative cycles of (m)FOLFIRINOX, those who had radiological response, and those with ypN0 disease. Adjuvant (m)FOLFIRINOX and other multiagent regimens were associated with prolonged OS, whereas an adjuvant single-agent regimen was not. The authors conclude that it cannot be ruled out that the association of adjuvant therapy with OS is weaker or even absent in certain subgroups, which deserves further study.
