Idecabtagene vicleucel (ide-cel) is an autologous BCMA-directed CAR T-cell therapy approved for relapsed or refractory multiple myeloma (RRMM) on the basis of results of the phase II KarMMa trial. A retrospective study by the Myeloma CAR T Consortium evaluated ide-cel for RRMM in the real world. Seventy-five percent of the patients would not have been eligible for participation in KarMMa because of comorbidities at the time of leukapheresis. Grade ≥ 3 cytokine release syndrome was seen in 3% of patient and grade ≥ 3 neurotoxicity occurred in 6%. Best overall and ≥ complete response rates were 84% and 42%. Median progression-free and overall survival were 8.5 months and 12.5 months, respectively. The authors conclude that safety and efficacy of ide-cel in patients with RRMM in the real-world setting were comparable with those in KarMMa.
