Approximately 40% of patients with diffuse large B-cell lymphoma (DLBCL) develops disease recurrence/progression after first-line R-CHOP. A real-world retrospective study at Anhui Medical University (Hefei, China) evaluated two years of lenalidomide maintenance among DLBCL patients with response to first-line R-CHOP. Lenalidomide maintenance was associated with significantly prolonged PFS (A), while OS was not significantly improved (B). The cumulative risk of CNS relapse was not significantly improved with lenalidomide (C). PFS was not different between the groups in patients with GCB type and not-GCB type in the lenalidomide group (D) or the control group (E). Within the lenalidomide group, PFS was better after complete response to R-CHOP than after partial response to R-CHOP (F). There was no difference between the groups in OS after disease progression (G). Lenalidomide maintenance was well tolerated. The authors conclude that after response of DLBCL to first-line RCHOP, lenalidomide maintenance was associated with improved outcomes.
