Real-world outcomes with enfortumab vedotin with or without pembrolizumab for advanced urothelial carcinoma in ultra-elderly patients

Enfortumab vedotin (EV) is indicated for treatment of locally advanced or metastatic urothelial carcinoma (la/mUC) as monotherapy and in combination with pembrolizumab (P). The ultra-elderly population, defined as patients aged at least 80 years, was underrepresented in prospective clinical trials with EV. A retrospective study at the University of California examined real-world outcomes with EV with or without P for la/mUC in ultra-elderly patients. The study included 29 patients. The primary endpoint was incidence and severity of EV treatment-related adverse events (TRAEs). At least one EV-related TRAE was observed in 79.3% of patients, but no grade 4 or 5 toxicities were observed. The objective response rate was 57.7% and the disease control rate was 88.5%. The median progression-free survival was 7.82 months and the median overall survival was 11.6 months. The authors conclude that this study did not identify excessive or unexpected toxicity from EV-based regimens in the ultra-elderly population.
