The anti-CD30 antibody drug conjugate brentuximab vedotin (BV) has demonstrated high response rates in patients with advanced-stage mycosis fungoides (MF) and Sézary syndrome (SS) with CD30 expression ≥ 10%. A multicenter retrospective study in Germany evaluated BV for advanced MF and SS patients with CD30 expression < 10%. The study population included 30 MF and two SS patients. The median number of prior systemic therapies was 3. BV was administered according to institutional standards. The overall response rate (primary endpoint) was 53.1%, including 4 complete responses and 13 partial responses. Stable disease was observed in 6 patients (18.8%), and progressive disease in 9 patients (28.1%). The median progression-free survival was 4.0 months (range 0.5-15.5) and the median time to next treatment was 7.25 months (2.0-15.5). The authors conclude that BV demonstrated encouraging activity in heavily pretreated patients with advanced MF/SS with low CD30 expression, achieving an objective response rate comparable to that observed in patients with higher CD30 levels.
