Radiotherapy versus inguinofemoral lymphadenectomy in vulvar cancer patients with sentinel node metastasis
Dr. Maaike Oonk
Introduction of the sentinel node (SN) procedure was a major advancement in vulvar cancer treatment. In SN-negative patients inguinofemoral lymphadenectomy (IFL) can be safely omitted. The multicenter phase II GROINS-V-II study found that inguinofemoral radiotherapy (50 Gy) is a safe alternative for IFL in vulvar cancer patients with sentinel node micrometastases, with minimal morbidity. For patients with SN macrometastases, radiotherapy with a total dose of 50 Gy resulted in more isolated groin recurrences compared with IFL.
