Idecabtagene vicleucel (ide-cel) is a BCMA-directed CAR T cell therapy. The multicenter phase 1 CRB-401 trial evaluated ide-cel for relapsed and refractory multiple myeloma in 62 patients. Post hoc 18-month follow-up of the trial found low rates of grade 3 or 4 cytokine release syndrome (6.5%) and neurotoxicity (1.6%) and https://media.springernature.com/full/springer-static/image/art%3A10.1038%2Fs41591-023-02496-0/MediaObjects/41591_2023_2496_Fig1_HTML.png?as=webp promising efficacy: the ORR was 75.8%; 64.5% achieved VGPR or better, and 38.7% achieved CR or stringent CR. Among exploratory end points, median duration of response, progression-free survival, and overall survival were 10.3, 8.8, and 34.2 months, respectively. Ide-cel expansion in blood and bone marrow correlated with clinical efficacy and postinfusion reduction of soluble BCMA. The authors conclude that these results confirm safety, tolerability, and efficacy of ide-cel.
