BCMA CAR-T celltherapy has show promise in treating relapsed or refractory multiple myeloma (R/R MM), but its long-term safety and efficacy are not clear. Pooled analysis of two clinical studies at Zhejiang University (Hangzhou, China) found that at a median follow-up of 20.2 months the safety profile was manageable. The objective response rate among evaluable patients was 94.8%, with 50.7% achieving a complete response. The 4-year progression-free survival and overall survival rates were 37.4% (95% CI 29.1-48.1) and 63.2% (54.8-72.8), respectively. Patients with a history of autologous stem cell transplantation and patients with extramedullary disease had worse responses and survival outcomes. Peak CAR-T cell expansion was positively correlated with ORR and CR. The authors conclude that BCMA CAR-T cell therapy for R/R MM resulted in significant and sustained responses, with a manageable safety profile.
