Alectinib is a first-line treatment for ALK-positive NSCLC. The optimal treatment after progression on alectinib is not clear. Pooled analysis of the phase 2 trials ALTA-2 and J-ALTA found that brigatinib had clinically meaningful activity after progression on alectinib, with ORR 31%, median DoR 9.2 months, median PFS 5.2 months, and median OS 25.0 months. Safety profile of brigatinib was consistent with previous reports.
