Low-grade serous carcinoma of the ovary or peritoneum is characterized by MAPK pathway aberrations and reduced sensitivity to chemotherapy relative to high-grade serous carcinoma. The randomized phase 2-3 GOG 281/LOGS trial compared the MEK inhibitor trametinib versus physician’s choice of standard of care in patients with recurrent low-grade serous carcinoma. The median progression-free survival was 13.0 months with trametinib versus 7.2 months with standard of care (HR 0.48; p<0.0001). The authors conclude that trametinib represents a new standard-of-care option for patients with recurrent low-grade serous carcinoma.
