Evidence suggests that stereotactic body radiotherapy might be a potent immunomodulator in advanced non-small cell lung cancer. A randomized phase 2 study at NewYork-Presbyterian and Weill Cornell Medical Center evaluated neoadjuvant durvalumab (anti-PD-L1) with or without SBRT for early stage NSCLC. Major pathological response was seen in 6.7% of patients in the durvalumab monotherapy group versus 53.3% in the durvalumab plus SBRT group (OR 16.0; p<0.0001). Neoadjuvant durvalumab plus SBRT was safe and well tolerated.
