Patients with relapsed or refractory diffuse large B-cell lymphoma (R/R DLBCL) progressing after CAR T-cell therapy have dismal outcomes. The prespecified post-CAR T expansion cohort of the multinational ELM-1 study evaluated the CD20xCD3 bispecific antibody odronextamab monotherapy for these patients. Objective response rate by independent central review was 48.3%, with complete response in 31.7%. Median duration of response was 14.8 months, and median duration of complete response was not reached after median follow-up of 16.2 months. Median progression-free survival and overall survival were 4.8 months and 10.2 months, respectively. CRS (no grade ≥ 3 events) was seen in 48.3% of patients; no ICANS was reported; grade ≥ 3 infections occurred in 20%. The authors conclude that odronextamab monotherapy demonstrated encouraging efficacy and generally manageable safety for R/R DLBCL after progression on CAR T.
