Multinational study of nadunolimab plus chemotherapy in patients with advanced or metastatic pancreatic cancer

Interleukin (IL)-1 pathway upregulation is implicated in pancreatic ductal adenocarcinoma (PDAC) progression and therapy resistance. Nadunolimab is an IL-1 receptor accessory protein (IL1RAP)-targetin antibody. A multinational study evaluated safety and efficacy of nadunolimab in combination with gemcitabine and nab-paclitaxel (GN) chemotherapy for previously untreated locally advanced or metastatic (LA/M) PDAC. The most frequent grade ≥ 3 adverse events was neutropenia (66% of patients). Infusion-related reactions occurred in 29% (grade 3 in 3%). Only one patient had grade ≥ 3 peripheral neuropathy. Median PFS was 7.2 months (95% CI 5.2-8.5) and median overall survival was 13.2 months (10.6-15.5) with one-year OS rate of 58%. Treatment efficacy was higher in patients with high versus low tumor baseline IL1RAP expression. The authors conclude that nadunolimab in combination with GN had promising efficacy and manageable safety among patients with previously untreated LA/M PDAC.
