Multinational retrospective study of outcomes with durvalumab consolidation for unresectable stage III NSCLC following incomplete cCRT

Many patients with unresectable stage III non-small cell lung cancer (NSCLC) may face challenges in receiving consolidation durvalumab followy concurent chemoradiotherapy (cCRT) due to incomplete cCRT or early disease progression. In the multinational retrospective POSITION study, among 2,076 patients, 23.0% had incomplete cCRT (group A), 12.7% experienced progression during or within 42 days after cCRT (group B), and 64.3% completed cCRT without progression. Median overall survival (OS) was 21.6 months (95% CI 17.2-28.0), 10.1 months (7.2-12.5), and 32.4 months (29.8-34.6) for groups A, B, and C, respectively, whereas landmark 60-months OS rates were 26.3%, 11.6%, and 33.2% for the three groups. Higher comorbidity burden and Black (versus White) race were associated with higher likelihood of incomplete cCRT. The authors conclude that in POSITION, 35.7% had incomplete cCRT or progressed during or within 42 days after cCRT, with unfavorable impact on OS.
