Interleukin-1 receptor accessory protein (IL1RAP) is essential for IL-1α and IL-1β signalling which leads to tumor progession and treatment resistance. A multinational retrospective study evaluated the combination of the IL1RAP targeting antibody nadunolimab and platinum doublet chemotherapy for advanced NSCLC, either as first-line therapy or as second-line therapy post-pembrolizumab. Median PFS was 7.2 months (95% CI 5.6-9.2) and median OS was 13.7 months (11.1-18.3) with 1-year OS-rate 54%. Greatest benefit was observed in patients with non-squamous histology treated in second-line post-pembrolizumab, with median OS 26.7 months and 1-year OS-rate 82%. Rates of grade 3 or higher neutropenia, anemia, and thrombocytopenia were higher than in previous trials of platinum doublet chemotherapy alone. The authors conclude that nadunolimab plus platinum doublet chemotherapy had promising efficacy in advanced NSCLC.
