Whether patients with Child-Pugh class B (CP-B) unresectable hepatocellular carcinoma (uHCC) benefit from active anticancer treatment versus best supportive care (BSC) is debated. A multinational retrospective study compared first-line immune checkpoint inhibitor (ICI)-based therapy versus best supportiv care (BSC) for these patients. In the IPTW populations the median overall survival was significantly longer in the ICI group than in the BSC group (7.50 months versus 4.04 months; HR 0,59; p<0.001). In multivariable analysis, ICI-based treatment was associated with a reduction of approximately 50% in the risk of death (HR 0.55; pxtagstartz0.001). The authors conclude that these findings provide comparative evidence of improved survival in association with ICI-based treatment compared with BSC in patients with uHCC with CP-B liver dysfunction.
