The multinational phase 3 DESTINY-Breast 02 trial compared trastuzumab deruxtecan (T-DXd) versus treatment of physician’s choice (TPC) for HER2-positive metastatic breast cancer (mBC) in patients previously treated with trastuzumab emtansine (T-DM1). Median progression-free survival was 17.8 months with T-DXd versus 6.9 months with TPC. T-DXd was tolerated, with grade 3 or higher treatment-emergent adverse events in 53% of patients. The authors conclude that T-DXd for HER2-positive mBC has a favorable benefit-risk profile, and can overcome resistance to T-DM1.
