The benefit of regional nodal irradiation in the treatment of breast cancer is well established for patients with pathologically positive axillary nodes. A multinational phase 3 trial evaluated regional nodal irradiation among patients with clinical stage T1-T3, N1, M0 who had ypN0 status after neoadjuvant chemotherapy. Among these patients, regional nodal irradiation did not decrease the risk of invasive breast cancer recurrence of death from breast cancer, and was not associated with improved disease-free survival or overall survival.
