Co-inhibition of PARP and androgen receptor activity might result in antitumor activity irrespective of alterations in DNA damage repair genes involved in homologous recombination repair (HRR). The multinational phase 3 TALAPRO-2 trial evaluated addition of the PARP inhibitor talazoparib versus placebo to first-line enzalutamide (androgen receptor blocker) for metastatic castration-resistant prostate cancer (mCRPC). Addition of talazoparib resulted in clinically meaningful and statistically significant improvement in radiographic progression-free survival both among all patients was well as among patients with HRR gene alteration status of deficient and among patients with HRR gene alteration status of non-deficient or unknown.
