Multinational phase 3 study of first-line cabozantinib plus atezolizumab versus sorafenib for aHCC

The multikinase inhibitor cabozantinib has shown promising clinical activity in combination with immune checkpoint inhibitors in multiple tumor types. The multinational phase 3 COSMIC-312 study compared cabozantinib plus atezolizumab versus sorafenib as first-line systemic treatment for advanced hepatocellular carcinoma. Cabozantinib plus atezolizumab resulted in longer PFS compared with sorafenib. Interim analysis of OS did not show a statistically significant benefit for cabozantinib plus atezolizumab versus sorafenib. The safety profile was manageable and consistent with the known safety profile of each agent.
