The multinational phase 2 PHERGain trial evaluated a chemotherapy-free treatment based on dual HER2 blockade with trastuzumab and pertuzumab in patients with HER2-positive early breast cancer (EBC) who were PET-responders after two cycles of treatment and had pathological complete response at surgery. Among these patients, the 3-year invasive disease-free survival rate was 94.8% (95% CI 91.4-97.1). There were no unexpected adverse events. The authors conclude that this strategy can identify about a third of patients with HER2-positive EBC, who could safely omit chemotherapy.
