Ifanatamab deruxtecan (I-DXd) is a B7 homolog 3-directed antibody-drug conjugate. The multinational phase 2 IDeate-Lung01 trial evaluated I-DXd for previously treated extensive-stage small cell lung cancer. Among patients receiving I-DXd 12 mg/kg every 3 weeks the median time to response was 1.4 months, the overall response rate was 48.2%, the median duration of response was 5.3 months, the median progression-free survival was 4.9 months, and the 9-month overall survival estimate was 59.1%. Any grade treatment-related adverse events were reported for 89.9%, grade 3 or higher TRAEs for 36.5%, TRAEs leading to discontinuation for 9.5%, and TRAEs leading to death for 4.4%. The authors conclude that I-DXd 12 mg/kg Q3W showed promising efficacy for previously treated ES-SCLC, with a safety profile consistent with previous reports.
