HER2 amplification has been detected in 2-3% of colorectal cancers. Trastuzumab deruxtecan is an antibody-drug conjugate of humanized anti-HER2 antibody with topoisomerase I inhibitor payload. The multinational phase 2 study DESTINY-CRC01 found promising and durable activity of trastuzumab deruxtecan in HER2-positive mCRC that was refractory to standard treatment. The ORR was 45.3% (95% CI 31.6-59.6). The safety profile was consistent with what was reported in previous trastuzumab deruxtecan trials. Interstitial lung disease and pneumonitis were fatal TRAEs in 2.6% of patients.
