Modakafusp alfa is a first-in-class immunocytokine directing interferon-α to CD38-positive cells. A multinational phase 2 trial compared modakafusp alfa monotherapy at two doses (120 and 240 mg every four weeks) for relapsed or refractory multiple myeloma (RRMM) in patients who had received a median of 6 prior lines of therapy. With 120 and 240 mg Q4W, the ORRs were 32% and 41%, and median PFS was 4.1 and 5.3 months, respectively. The most common treatment-related adverse events were thrombocytopenia and neutropenia. The authors conclude that modakafusp alfa monotherapy had activity in patients with RRMM.
