Multinational phase 1b-2 trial of epcoritamab plus GemOx in transplant-ineligible relapsed/refractory diffuse large B-cell lymphoma

The multinational phase 1b-2 NHL-2 trial evaluated the combination of epcoritamab (CD3xCD20 bispecific antibody) plus gemcitabine-oxaliplatin (GemOx) for relapsed/refractory diffuse large B-cell lymphoma (R/RDLBCL) in transplant-ineligible patients (median age 72 years; 62% ≥ 2 prior therapy lines). The overall response rate was 85%, with complete response in 61%. The median duration of complete response was 23.6 months, and the median overall survival was 21.6 months. Common treatment-related adverse events were cytopenias and cytokine release syndrome (52% overall; 1% grade 3) not resulting in discontinuation. The authors conclude that epcoritamab plus GemOx yielded deep, durable responses and favorable long-term outcomes in transplant-ineligible R/R DLBCL.
