Multicenter study of long-term outcomes of alternative donor allogeneic transplantation

A donor search prognosis score predicts the likelihood of a patient finding a fully HLA-matched unrelated donor (MUD). Patients without a MUD may use alternative donors. The multicenter BMT CTN 1702 trial (n=1,751) compared 2-year survival from beginnin of the search between patients Very Likely (>90%; 54.7% of patients), Less Likely (10%-90%; 29.5% of patients), and Very Unlikely (<10%; 15.8% of patients) to find a MUD. Survival and transplant outcomes did not differ between the three search prognosis groups. The authors conclude that using a donor search prognosis strategy to prioritize an alternative donor for patients Very Unlikely to find a MUD resulted in outcomes that were not statistically different compared with those Very Likely to find a MUD.
