A multicenter study in the USA evaluated daily exemestane for 21 to 42 days as pre-surgical intervention for prevention of endometrial cancer (EC) in postmenopausal women undergoing hysterectomy for endometrial inraepithelial neoplasia (EIN) or low-grade EC. Primary endpoint was change in proliferation, measured by Ki-67 expression in pre- and post-treatment endometrial tissue specimens. Median Ki-67 score decreased from 40.7% (IQR 8.8-13.8) at baseline to 18.1% (8.8-31.8) at surgery, representing a median absolute change od 20.4% (-29.6 to -6.7; p<0.001). The authors conclude that in this pilot study, exemestane demonstrated anti-proliferative effects in EIN or low-grade EC.
