A retrospective study at 5 centers in Canada and the USA evaluated outcomes of stereotactic ablative radiotherapy (SABR) for oligometastatic tumors of endometrium, ovary, cervix, and vulva or vagina in 250 patients with 320 lesions between 2011 and 2020. At 5 years after SABR, cumulative incidence of local recurrence was 18.5%, cumulative incidence of distant recurrence was 73.1%, and overall survival was 33.1%. Median chemotherapy-free survival was 21.7 months. Toxic effects were minimal. The authors conclude that SABR appeared to have excellent local control, and certain patients may achieve durable distant control and overall survival as well. It may be possible to delay time to chemotherapy in select patient subtypes.
