A multicenter retrospective study in the USA investigated real-world efficacy and safety of lisocabtagene maraleucel (liso-cel) in patients with relapsed or refractory large B-cell lymphoma (R/R LBCL), including those with high-risk disease, secondary CNS involvement, comorbidities, and poor fitness. In the Center of International Blood and Marrow Transplant Research Registry (Feb 2021 to Feb 2025) the authors identified 1116 patients receiving liso-cel and having had ≥ 1 effectivity and safety assessment after infusion, including 195 in the second-line setting, 71 with secondary CNS involvement, and 257 with transformed LBCL Median age was 71.1 years (range 21.5-91.2); 6.6% had ECOG performance status ≥ 2; 53.4% had ≥ 1 comorbidity, and the median number of prior therapy lines was 3 (range 1-16). Among 1109 efficacy-evaluable patients, objective response rate was 81.2% with complete response in 71.3%. CRS was reported for 51.9%, with grade ≥ 3 events in 2.5%; ICANS was reported for 26.6%, with grade ≥ 3 events in 9.2%. The 12-month nonrelapse mortality rate was 6.1% (95% CI 4.6-7.8). Twelve-months rates of duration of response, progression-free survival, and overall survival were 60.2% (95% CI 56.4-63.9), 51.2% (48.0-54.4), and 67.6% (64.5-70.6), respectively. The authors conclude that these real-world data confirm the efficacy and safety of liso-cel for R/R LBCL.
