Multicenter retrospective real-world analysis of teclistamab for relapsed and refractory multiple myeloma in Germany

Teclistamab, a BCMA x CD3 bispecific antibody, has shown high response rates and durable remissions in the MAJESTIC-1 trial of patients with relapsed and refractory multiple myeloma (RRMM). The real-world outcomes with teclistamab for RRMM are not yet clear. A retrospective analysis of RRMM patients of 18 German centers included 123 patients with triple-class (93%) or penta-drug (60%) refractory disease. The figure shows that with a median follow-up of 5.5 months, the overall response rate was 59.3% and the median progression-free survival was 8.7 months, while median duration of response and overall survival were not reached. The ORR and median PFS were significantly lower in patients with extramedullary disease (37%/2.1 months), ISS 3 (37%/1.3 months), and patients previously treated with idecabtagene vicleucel (33%/1.8 months). Infections and grade 3 or higher cytopenias were the most frequent adverse events. The authors conclude that teclistamab exhibited a comparable efficacy and safety profile in the real-world setting as in the pivotal MAJESTIC-1 trial.
