Multicenter randomized trial of survival impact of prechirurgical peritumoral lidocaine infiltration in early breast cancer
Dr. Rajendra Badwe
There is preclinical and clinical evidence of surgical dissemination leading to metastases. Surgery-induced acute hypoxia can activate prometastatic pathways through voltage-gated sodium channels, which can be blocked by local anesthetics. A multicenter randomized trial in India evaluated peritumoral infiltration of lidocaine before surgery in early breast cancer. Compared with the control group, the lidocaine group had significantly better disease-free survival (HR 0.74; p=0.017) and overall survival (HR 0.71; p=0.019).
