Multicenter randomized trial of radiation therapy with or without cisplatin for locally recurrent endometrial cancer

Pelvic recurrence is a frequent pattern of relapse among women with endometrial cancer. A multicenter randomized trial in the US evaluated addition of cisplatin to radiation therapy (RT) for these women. Patients were randomized 1:1 to RT plus concurrent cisplatin (40 mg/m2 every week) or RT alone. Rates of acute toxicity were higher in patients treated with chemoradiation as compared to RT alone. Median progression-free survival was numerically longer among patients receiving RT alone (not reached) as compared to RT plus cisplatin (73 months; HR 1.25; 95% CI 0.75-2.07). At 3 years, 73% of RT alone patients and 62% of RT plus cisplatin patients were alive and free of disease progression. The authors conclude that excellent outcomes can be achieved for women with localized recurrences of endometrial cancer when treated with RT.
