A multicenter randomized phase 2 trial in the United States evaluated the efficacy of prophylactic radiation therapy (RT) to asymptomatic high-risk bone metastases in preventing skeletal-related events (SREs). RT compared with standard of care reduced SRE at 1 year from 29% to 1.6%. The median overall survival was 1.7 years in the irradiated group versus 1.0 years in the control group (HR 0.49; p=0.018), which maintained significance in adjusted analysis.
