The multicenter randomized phase 2 MGMT-NET in France evaluated alkylating agents (ALKY) and oxaliplatin for advanced neuroendocrine tumors, with either proficient MGMT status (pMGMT-NET) or deficient MGMT status (dMGMT-NET). The primary aim was to detect a difference of 35% between the 3-month objective response rate in pMGMT-NETs versus dMGMT-NETS when treated with ALKY. This endpoint was not reached: the 3-month ORR was 29.4% versus 8% (OR 3.5; p=0.172). However, best ORR (52.9% versus 11.5%) and median progression-free survival (14.6 versus 11.3 months) were better for dMGMT-NETs versus pMGMT-NETs. MGMT status did not affect oxaliplatin effectivity. The authors conclude that ALKY has clinical activity in patients with dMGMT-NETs.
