The proteasome inhibitor bortezomib was shown to be effective in patients with relapsed T-cell acute lymphoblastic leukemia (T-ALL) and T-cell lymphoblastic lymphoma (T-LL). The multicenter phase 3 AALL1231 trial by the Children’s Oncology group evaluated addition of bortezomib to a modified augmented Berlin-Frankfurt-Münster backbone during induction and delayed intensification for newly diagnosed T-ALL and T-LL in children and young adults. Addition of bortezomib resulted in improved event-free and overall survival among the T-LL patients. Systemic therapy intensification allowed elimination of prophylactic cranial radiation in more than 90% of T-ALL patients without excess relapse.
