A multicenter phase 2 trial in the USA evaluated perioperative nivolumab with (arm A) or without (arm B) ipilimumab for resectable diffuse pleural mesothelioma (DPM). The trial met its primary endpoints of feasibility and safety. In arms A and B, 81.3% and 85.7% proceeded to surgery, respectively. In each arm only one dose-limiting toxicity was seen. The efficacy of the treatments was promising. Patients with detectable ctDNA on cycle 3 and pre-surgery had shorter PFS. The authors conclude that these results support the feasibility of perioperative immune checkpoint blockade and the clinical utility of ctDNA analyses to capture residual disease in resectable DPM.
