Multicenter phase 2 trial of neoadjuvant chemotherapy plus dual immune checkpoint inhibition for advanced-stage ovarian cancer

The multicenter phase 2 TRU-D trial in South Korea evaluated three cycles of neoadjuvant chemotherapy combined with durvalumab and tremelimumab for unresectable stage III-IV epithelial ovarian cancer (EOC). After debulking interval surgery patients received three cycles of durvalumab plus adjuvant chemotherapy, followed by 12 cycles of durvalumab as maintenance therapy. The 12-month progression-free survival rate was 65.9% (95% CI 52.8-NE), while the 24- and 30 month PFS rates were 38.6% (26.7-NE) and 36.4% (24.7-NE), respectively. Pathological complete response was seen in 11.1% of patients. The 30-month overall survival rate was 87.7%. The most common grade≥ 3 adverse event was neutropenia, in 26.7% of the patients. The authors conclude that neoadjuvant chemotherapy combined with durvalumab and tremelimumab is feasible and effective for advanced-stage EOC.
