Multicenter phase 2 trial of local radiotherapy and MRD-guided ofatumumab for early-stage follicular lymphoma

The mainstay of treatment for early-stage follicular lymphoma (FL) is local radiotherapy. The phase 2 FIL MIRO trial, at 27 centers in Italy, found that involved-field radiotherapy resulted in MRD-negative status in 40% of patients. After ofatumumab consolidation (8 weekly doses of 1000 mg) among MRD-positive patients, 92% of these patients became MRD-negative. After a median follow-up of 46.1 months, 61% of these patients remained in complete response and were MRD-negative. The authors conclude that local radiotherapy is frequently not associated with the eradication of FL, and ofatumumb consolidation resulted in molecular remission in almost all patients.
