The combination of immune checkpoint inhibitors and antiangiogenic agents can have synergistic antitumor activity. A phase 2 trial at 16 centers in China evaluated the combination of camrelizumab and famitinib for previously untreated advanced or metastatic NSCLC with PD-L1 tumor proportion score ≥ 1% (wild-type EGFR and ALK). Among 41 patients, the combination achieved an objective response rate (primary end point) of 53.7% (95% CI 37.4-69.3) and a disease control rate of 92.7% (80.1-98.5). The median progression-free and overall survival were 16.6 months (95% CI 8.3-not reached) and not reached, respectively, with an estimated 12-month OS rate of 76.8% (60.0-87.3). Grade 3 or higher TRAEs were hypertension (22% of patients), increased alanine aminotransferase (12%), decreased neutrophil count (10%), proteinuria (7%), decreased platelet count (7%), and hypokalemia (7%). One patient died from hemoptysis, which was considered possibly related to the study treatment. The authors conclude that the combination of camrelizumab and famitinib had promising antitumor activity and acceptable safety as first-line treatment for aNSCLC with PD-L1 TPS ≥ 1%.
