The current first-line treatment for advanced triple-negative breast cancer (aTNBC) is determined by the expression of PD-L1. The phase 3 ATRACTIB trial, at 22 centers in four countries, evaluated the first-line combination of atezolizumab with paclitaxel and bevacizumab for aTNBC, independently of PD-L1 status. The trial included 100 female aTNBC patients, with most evaluable tumors being PD-L1 negative (97.6%). The median progression-free survival was 11.0 months (95% CI 9.0-13.4), meeting the primary endpoint. The objective response rate was 63.0% (95% CI 52.8-72.4) and the median overall survival was 27.4 months (23.4-37.4). There were no treatment-related deaths or new safety signals. The authors conclude that the first-line combination of atezolizumab with paclitaxel and bevacizumab demonstrated significant antitumor efficacy for aTNBC patients.
