Standard treatment for unresectable, locally advanced non-small cell lung cancer (LA NSCLC) is durvalumab after concurrent chemoradiotherapy (CRT); however, 20% to 30% of patients do not receive durvalumab because of adverse events during CRT. The phase 2 DOLPHIN trial, at 12 centers in Japan, evaluated the chemotherapy-free regimen of durvalumab plus concurrent radiotherapy (60 Gy) followed by durvalumab maintenance for PD-L1 positive LA NSCLC. Among 35 patients the 12-month progression-free survival rate was 72.1%. Scheduled radiotherapy was completed in 97.1% of patients, and pneumonitis or radiation pneumonitis of grades 3 or 4 occurred in 11.8% of patients. Grade 5 AEs occurred in 2 patients (5.9%). The authors conclude that durvalumab plus concurrent radiotherapy for unresectable LA NSCLC had promising efficacy with tolerable adverse events.
