Although the treatment of iatrogenic and HIV-related Kaposi sarcoma is well defined and mostly based on restoring the immune function, the treatment of classic and endemic Kaposi sarcoma is less well established. A multicenter phase 2 study in France evaluated pembrolizumab in patients requiring systemic treatment for classic or endemic Kaposi sarcoma with cutaneous extension. Twelve percent of patients had complete response and 59% had partial response as best overall response (BORR 71%; 95% CI 44-90) while the other 29% had stable disease. The safety profile was acceptable.
