Multicenter phase 1b trial of frontline magrolimab plus azacitidine for higher-risk myelodysplastic syndromes

Magrolimab is a monoclonal antibody that blocks cluster of differentation 47, a don’t-eat-me signal overexpressed on cancer cells. A multicenter phase 1b trial in the US evaluated magrolimab plus azacitidine for patients with previouly untreated higher-risk myelodysplastic syndromes (HR-MDS). The combination was well tolerated. Complete response was seen in 33% of patients and overall response in 75%. Median duration of overall response and progression-free survival was 9.8 months and 11.6 months, respectively. Median overall survival was not reached during 17.1 months follow-up. Among the patients with TP53 mutations (26%), 40% achieved complete response with median overall survival of 16.3 months. The authors conclude that the combination had promising efficacy in patients with untreated HR-MDS, including those with TP53 mutations.
