Multicenter phase 1 trial of donor-derived anti-CD19 CAR T cells for relapsed or refractory B-cell ALL after allogeneic HSCT

Chimeric antigen receptor-modified T cell therapy has been widely reported as improving the outcomes of B-ALL. No studies of feasibility and safety of donor-derived CAR T cells for relapsed or refractory B-ALL after alloHSCT have been published. A phase 1 study at two centers in China included nine patients who received a single infusion of donor-derived CAR T cells (‘GC007g’). At four weeks, all patients had CR/CRi with undetectable MRD. At a median follow-up of 475 days, seven patients remained in CR/Cri. The overall response rates were 100% at 3 months, 88.9% at six months, and 75% at twelve months. One-year progression-free and overall survival rates were 77.8% and 85.7%, respectively. The authors conclude that among patients with R/R B-ALL after alloHSCT, donor-derived CAR T cells induced durable remissions with manageable safety profiles.
